The checklist below has been designed to
assist you as you go through the enrollment process.
1. Getting to know us
* Visit our website at www.woodburylutheran.org/school.
* Call to speak to the Director, schedule a personal tour of our facility or attend our Open House on February 23rd from 5:30-7:00 PM.
* Obtain enrollment forms from our school office weekdays from 8:00 A.M.-4:00 P.M., or download forms at the following links:
2. Complete and return enrollment form and registration fee.
* Submit fully completed form with non-refundable registration fee to our school office or mail to:

*All families will receive an e-mail confirmation of their child's class day/time.
*Online enrollment is not accepted.
A student information packet will be mailed in March/April prior to the school year. Please file this packet of student information forms in a safe and easily accessible place!
3. Return completed student information forms below on dates requested (August 1st / Preschool and June 1st / Kindergarten).
* Child Personal Information forms/ * Release / Consent Forms/ * Signed Health Care Summary and Immunization Form/ * Child Emergency Card
4. Call to schedule an appointment with the Director to discuss your child's allergies, health concerns, or special needs.
* In order to provide a safe, positive, beneficial early learning experience, we need to be educated regarding your child's health concerns or special needs. Important forms must be completed by your child's health care provider prior to beginning school.
5. Additional forms below will be sent prior to the start of school, given at Orientation in the fall, or at time of late enrollment.
